HOUSE SHIRT ORDER FORM

House Shirts are $13.75 for Short Sleeve and $19.75 for Long Sleeve. Please indicate your name, your son's name, and house. The cost will be charged to the student account through Smart Tuition. For the safety of the students, shirts will be placed in a bag, then given to the homeroom teacher to hand out to the student.
